The most critical time for an individual’s recuperation is when he or she returns home from the hospital. At the Y, we provide individuals the support, guidance and resources needed to achieve greater health and well-being in spirit, mind and body.
Brooks Rehabilitation is proud to offer several different adaptive wellness programs in partnership with the Y. Each program is designed to help those with mobility impairments through supervised physical activity. Through these programs, participants learn how to stay active and improve their health after they complete their formal physical therapy.
Each program requires a registration. All programs require a referral and a diagnosis specific evaluation from a Brooks neuro physical therapist. The evaluation is sent to the staff that oversees the program. You will then complete an orientation and receive a customized and supervised exercise plan based off of your goals and needs. Programs are created for each participant by rehab experts from Brooks Rehabilitation Hospital in collaboration with Y Staff. Participants are asked to check in at the Welcome Center before attending a class. The diagnoses we serve in this program are Stroke, Brain Injury, Parkinson’s Disease, and Multiple Sclerosis.
Adaptive Community Wellness
Brooks Rehabilitation is proud to offer Adaptive Community Wellness in partnership with the Y. This program encompasses all diagnoses of Stroke, Brain Injury, Parkinson’s Disease, and Multiple Sclerosis. A specialized exercise plan is created for each participant under the guidance of exercise specialists.

Multiple Sclerosis Wellness and Brain Injury Wellness
Multiple Sclerosis (MS) Wellness is an exercise program designed for those living with MS. During this program, participants will learn how to properly use equipment such as hand weights and resistance bands. A specialized exercise plan is created for each participant under the guidance of exercise specialists. The exercises are designed for all ages and will be modified for participants regardless of physical activity level.
Brooks Rehabilitation is proud to offer a brain injury wellness program, in partnership with the Y, to help survivors of brain injuries with supervision, motivation and support. A specialized exercise plan is created for each participant under the guidance of exercise specialists.

Parkinson’s Wellness
This program is designed to help participants maintain and improve their strength and endurance using gym equipment. The class fosters social support that comes from moving with a group of people all familiar with what it means to live with Parkinson’s. An exercise physiologist facilitates this class. An initial evaluation by a Brooks Rehabilitation therapist is required prior to beginning this program.

Stroke Wellness
Brooks Rehabilitation is proud to offer a stroke wellness program, in partnership with the Y and Halifax Medical Center, to help stroke survivors with supervision, motivation and support. The Stroke Wellness Program provides patients with customized exercise programs and nutrition education to support the goal of restoring strength, endurance and agility after a stroke.
